Call 515-290-9934 to schedule an appointment for a fall risk assessment to help identify your risks for falling.

The assessments are coordinated by the ISU Walk with Ease program in collaboration with Story County Extension and the Physical Therapy program at Mary Greeley Medical Center. Individuals will complete a brief risk survey followed by three established balance/function tests. The assessment takes only 15 minutes and will provide a personalized report and recommendations. 

Please wear comfortable clothing and shoes that will allow easy movement.
