When discussing Diabetes Prevention Program with provider realized that prior to implementation a Grand Rounds was presented. Since that time our program is nationally recognized by the CDC for both regular participants and by CMS for Medicare Diabetes Prevention Program. Also we received a grant for a app that allows us to participate for 6 months with immediate feedback to participants at no extra cost. Thus we felt we should again circle back and educate again.
At the conclusion of this activity, learners will be able to:
- Identify the qualifying criteria for the Medicare Diabetes Prevention Program.
- Discuss the goals and benefits for participants of MGMC Diabetes Prevention Program.
- Explain how to refer t the MGMC Diabetes Prevention Program.
- Describe the Fresh Tri app that MGMC Diabetes Prevention participants enrolled from January-March 2021 will have access to use for one year.
