Israel Family Hospice Garden Bricks & Recognition Plaques

We are deeply grateful to the many families who have honored their loved ones at the Israel Family Hospice House over the past 26 years through memorial bricks in our garden and recognition plaques throughout the building. We know how meaningful these tributes are. If you would like to be contacted about retrieving a memorial item, please complete the form below. We appreciate your patience and connection to the Israel Family Hospice House.

Please refer to this FAQ for more information regarding Mary Greeley’s Hospice program and the decision to close the Israel Family Hospice House.
