Medical Records

The Mary Greeley Medical Center Medical Records office is the main repository for medical information about a patient's hospital visits. These files help determine prior conditions or issues that may affect your current or future treatment at Mary Greeley Medical Center. The Medical Records office's responsibilities include:

  • Transcription (digital dictation and typing of physician notes)
  • Coding (diagnostic coding for insurance or Medicare billing purposes)
  • Assembly and Analysis (checking for elements necessary for completion of your file)
  • Clerical (reception, chart flow and completion)

The safety and confidentiality of your medical information is of the utmost importance. Your personal information and medical documentation is protected under federal HIPAA regulations.

Medical records are retained on file for 10 years, per Iowa law.

Request Medical Records

In accordance with HIPAA regulations, copies of medical records can be obtained by calling 515-239-2046, Monday through Thursday, 8 a.m. to 3:30 p.m., and 515-230-2141, Friday, 8 a.m. to 3:30 p.m.

You may also download an authorization for release form and return to us via e-mail or fax, 515-239-2049. The release form must be filled out in its entirety, and signed in order to be processed.
